Sample Onboarding Check list
Below is a list of items that you, as the client, are responsible for providing within the first 45 days of onboarding. If any item is not applicable to your organization, please let us know so we can avoid searching for the information.
Contracting
- Copies of all active contracts
- Any fee schedules available
- List of health plans out of network that the group would like to enroll
- List of contracts to obtain
- Letterhead with permission for exydoc to obtain contracting and credentialing information from health plans & work on the organization’s behalf
Credentialing
- Provider roster
- Inclusive of practice locations, phone & fax numbers, working days/hours, telehealth (y/n), current insurance plans. Must have social security numbers listed, Medicare and Medicaid ID’s (if applicable)
- Group Info
- W9
- Group liability insurance
- Any group ID numbers (Medicare/Medicaid)
- Individual Provider info
- CAQH logins
Other Items
- Creation of users in health plan sites such as Availity, UHC, etc.

- Administrator information for PECOS (if Medicare participating)

Sample Timeline*
- exydoc: Onboarding Call – 3/12/2025
- Client: Roster: Group, Service Location, Provider – On or before the week of 3/31/2025
- exydoc: Status Call – Week of 4/2/2025
- Client: Upload Files – 3/12/2025 to 3/31/2025
- Contracts – Before 3/31/2025
- W9 – On or before the week of 3/31/2025
- Liability – Week of 3/31/2025
- Any Supporting Files
- exydoc: Schedule Training – TBA
- Post Onboarding: After training, you will be responsible for uploading new information and updating existing data directly on the exydoc platform.
*Missing these deadlines will result in significant delays to your onboarding with exydoc, impacting both access and the overall timeline. Additionally, any information not provided within the first 45 days will need to be entered directly into exydoc by you, the client. Access to the shared Teams channel will also be revoked.
